Use this guide to complete IRS Form W-8BEN, which is required for individuals who are not U.S. citizens or residents for tax purposes.

Note: If you are a U.S. entity or person, do not use this form—please refer to Form W-9 in app.


Part I: Identification of Beneficial Owner

Line 1 – Full Legal Name

Enter your full legal name as it appears on official documents.


Line 2 – Country of Citizenship

🔴 Required
Enter your country of citizenship. If you have dual citizenship, choose the country where you currently reside and are a citizen.

Important: If you are a U.S. citizen or U.S. resident for tax purposes, do not fill out this form.


Line 3 – Permanent Residence Address

🔴 Required
Enter your full residential address (no P.O. boxes allowed). This must be in the country of citizenship listed on Line 2.


Line 4 – Mailing Address

🔴 Required (if different from Line 3)
Enter your mailing address, if different from your permanent address. Leave blank if the same as Line 3.

Note: P.O. boxes are not accepted.


Line 5 – U.S. Tax Identification Number (SSN or ITIN)

Leave blank.


Line 6a – Foreign Tax Identifying Number (FTIN)

🔴 Required (if you reside outside Canada)
Enter the FTIN issued by your country of residence.

Do not enter your Canadian SIN.


Line 6b – No FTIN Available

Check this box only if your country does not issue FTINs.

You can verify this on the IRS website.


Line 8 – Date of Birth

🔴 Required
Format: MM/DD/YYYY


Part II: Claim of Tax Treaty Benefits

Line 9 – Country of Residence for Tax Purposes

🔴 Required
Enter the name of the country where you are a tax resident.


Line 10 – Special Rates and Conditions

Leave blank unless specifically instructed otherwise.


Part III: Certification

Tick Box

Only check this box if you are signing on behalf of the person listed on Line 1.


Signature of Beneficial Owner

🔴 Required
Print the form, sign by hand.

Digital signatures are accepted.


Date

🔴 Required
Enter the date the form is signed (MM/DD/YYYY).


Print Name of Signer

🔴 Required
Clearly print the name of the individual signing the form.
